Certified copies of most Missouri birth and death records may be obtained through our office by filling out a request form. We can access the following records:

• Births from 1920 to present
• Deaths from 1980 to present

***AS OF MARCH 1,2011, YOU WILL NEED THE FOLLOWING**

One primary identification document that includes your name and photo such as: a driver’s license, U.S. military ID card, U.S. passport, work or school ID, etc.

OR

At least two alternate forms of identification (Documents must contain your name and the name of the company or organization issuing the document.)

A $15.00 search fee is required when requesting a birth record. A 5-year search may be conducted. This fee entitles the applicant to one (1) certified copy if the record is found. For additional copies of the same record, applicant should submit a fee of $15.00 for each.

A $13.00 search fee is required when requesting a death record. A 5-year search may be conducted. This fee entitles the applicant to one (1) certified copy if the record is found. For additional copies of the same record requested at the same time for the same person, applicant should submit a fee of $10.00 for each.

For expedited service, contact Vital Chek toll-free at 877-817-7363 twenty-four hours a day, seven days a week. Documents can be requested online or by phone and charged to a major credit card. However, there is an additional $9.95 charge for the use of this service. To further expedite the request, overnight services are offered.
